Overview
Create a Webform in Oases to post a form on your website to accept new customer enrollments directly to your Oases database.
This form may be used independently on your website or linked to your MarketPlace to gather additional information from registrants.

Information & Records Created
The Customer Enrollment Webform will create a new customer record in your desired destination program and create and link up to 4 students. The students will come in with an inactive status unless used in conjunction with a sale in MarketPlace for an upcoming session.
All data except the following will flow to the newly created customer record.
Student fields will create linked student records:
- Student Last Name
- Student First Name
- Grade
- Address that will copy from the Customer address
- Cell
- Birthday
- Gender
- Student Miscellaneous fields 1 through 5
The new customer record may be viewed by requesting to see Inactive Customers in the customer list.
Concierge Service
When transitioning to a new platform the forms you have on your website are often the items you need to switch out early, while you are not familiar with your new Oases platform. To save time while you are learning Oases we can create Custom Fields and your new Webform for you as part of our paid Concierge Service.
Concierge is billed at $50.00 per hour. Submit a support ticket with the name, type and fields required for your new Webfrom and we will quote to build it for you.
Create New Customer Webform
1. In Oases select: Setup > Other > Custom Webforms.
2. At the top of the page select Add New Webform (new customers can take over the sample webform by pressing the pencil).
3. In the pop up box define the settings for your new form:
- Select a Type of Customer.
- Name your form.
- Add any Tags you would like attached to the customer record created by Oases.
- Select the destination program. If you have multiple programs we recommend creating a new program to receive your online applications to keep them separate but this is not required.
- URL of the page to show your customer after submission.
- Email address for notifications.
Create First Field
4. You can create your first field from the set up field or save and create it later.
5. Choose the field type for the first field of your new form. The first field will be the first name in a text field.
6. Name the field and set the Destination field in Oases.
If you would like a red asterisk for required fields add the following code to your Field Name after your name text.

7. Press Save.
Add Sections
8. If required press Add New Section to add a section header. This can be used as a section header or to give instructions to the applicant. Press Save and move into position in the form.
Add Additional Fields
9. Add more fields to your form as required. Add the last name field and your address fields.
The first fields must be the first name and the last name. The form must also include the street address.
10. The way you build your fields must match the fields in Oases so the address for example will be built from separate fields as shown above. As the State in Oases is a drop-down so the State code will be a drop down type of field in the form. When you select State as a destination you don't have to type in all the States as options just select fill with States.
Verifying an Email Address
11. You will need two email fields to add an email and verify it. You may request and verify one email address per form only.

Drop Down Menus
12. If you have a drop-down menu in your application it must match the drop-down menu in Oases EXACTLY, whether it is one you created in your custom fields or one of the existing drop-downs.
13. Program your drop-down responses exactly the same as you programmed your custom field responses or the existing Oases responses, adding the answers you want the applicant to select from as shown below.
14. The 'exact match' criteria includes capitalizations and symbols.
Request Student Names & Information
15. Add a new section and create fields to bring in student names and grades to create up to 4 inactive student records attached to the customer. You can have a student first name, last name, grade, birthday, cell phone, email and the 5 misc fields for up to 4 students on this form. A student image may not be uploaded via the customer form.
16. When creating the Student Grade select Combo and auto fill with the student grades/years from your database.
17. At least one student first and last name must be included on your form.
Uploading Documents
18. One document upload field may be used per form. This allows for 3 documents of up to 20MB each to be added and these will go to the document page of the customer record.

Import Customer Image
19. When used as a stand-alone Webform the Customer Webform can import an image that Oases will display in the customer record in the image location. This field cannot be added to a Customer Webform that will be allocated as a MarketPlace WebfForm.
To import student photos the Student WebForm must be used.
Final Section
20. Consider using a final section at the bottom of your form with additional instructions. as exampled below.
Publish
21. Press the link icon at the top of your form and then select Copy to Clipboard.
22. Publish this code on a new page of your website or pass this to your web-master.
If you don't have a website copy the direct link to email to prospects/applicants.
23. If you make a change to your form in Oases you do not need to re-publish the code snippet. It will update dynamically.
View Published Form
24. You may now view your form on your new website page. Test your form before adding it to a live menu on your site making any changes as required in Oases.
Notification
25. Oases will email the address you set when the form is submitted.
26. As any students will come in as inactive, view the new customer record by opting to view inactive records.
